Four Steps to Your New Passport
Gather Documents
Collects all your required documents — see the Passport Documents List below.Take your documents to the Post Office
At the Post Office you will sign the Passport Application (DS-11) in front of an authorized agent.You will be given a sealed envelope.
(Please … do not open it.)Drop Off or overnight deliver the sealed envelope to us at Prompt Passport Service.
View this PDF for addresses and fees.The Prompt Passport Agent hand-delivers the sealed envelope to the regional passport agency processing.
Once ready, the passport is either:
Picked up in person by us
orShipped directly to you via 1–2 day shipping with tracking.
New Passport Documents List
Gather these items before your post office appointment for a new passport.
1) Photocopy of a Valid Drivers' License or State ID Front and Back
If your ID was issued in the last 6 months or is expired you will need to provide at least 2 additional IDs:
- In-state, fully valid non-driver ID (without photo)
- Learners or Temporary Driver Permit (without image)
- Temporary driver’s license (without image)
- Social Security card
- Voter registration card
- Employee work ID
- Student ID
- Medicare or health card
2) Evidence of Citizenship - Must provide 1 of the following
- Fully valid, undamaged U.S. Passport (can be expired)
- Original or certified copy of your U.S. birth certificate
- Certificate of Naturalization
3) Passport Fees -
- $212.05 payable to the U.S. Department of State for a U.S. Passport book
- $242.05 payable to the U.S. Department of State for U.S. Passport Book & Card

4) Passport Photos - CVS, Walgreens, or Post office
- In color
- No glasses
- Taken within the last 6 months
- In front of plain white or off-white background
- Neutral facial expression with both eyes open
- Do not wear a hat or head covering that obscures the head or hairline
5) Travel Itinerary • 2 copies , the 2nd copy is for us.
One copy is for the Post office. The extra copy is put on the side for us.
- Print the Flight itinerary emailed to you by your travel provider.
- Hotel Reservations (Can only be used for driving across the border) Mexico or Canada
- Business Travelers ( Will need a Letter from here employer on company letterhead stating the reason for emergency travel)
6) Letter of Authorization • 2 copies , the 2nd copy is for us.
7) Passport Application
The Passport Application (form DS-11) is created online, the link is below.
- Print 2 copies of your application when completed
- Don’t sign the application. It must be signed in front of the Passport acceptance agent at the Post office
